被邀請人對上海市相關防疫措施知情書
為做好本市新冠肺炎疫情防控工作，保障您的身體健康和生命安全，在華期間請您遵守我國相關法律法規，配合落實相關防控措施，現就相關事宜告知如下🚣🏼♀️：
一、請主動如實告知在滬邀請單位您入境中國前14天的旅居史和個人健康狀況，擬入境的交通方式，以及航班號或火車♜⛳️、輪船班次等信息🙆🏿，配合邀請單位做好防控預案。入境後🧑🏽🎓，按照國家和本市有關防疫要求進行隔離👂🏿、檢測等健康管理。
二🦸🏼♀️、請每日進行體溫監測🧑🍼，在公共區域請自覺佩戴口罩✍️，不至人群聚集場所活動，保持一定的社交間距，做好工作、生活環境的日常清潔，保持室內空氣流通。
三、在華期間如出現身體不適就醫時，請主動告知醫生14天內的旅居史，接觸人員等信息🕺™️，配合做好流行病學調查。一旦確診為新冠肺炎🧑🏻🍼，請保持良好心態，加強與醫護人員溝通，積極配合治療。
四、請嚴格遵守《中國人民共和國傳染病防治法》等相關法律和上海市政府疫情防控的相關規定🏅。如有拒絕執行，甚至擾亂防疫秩序，危害公共健康和安全的行為，上海市有關意昂4將依法追究法律責任🧗🏼♀️。

Informed Letter of COVID-19 Prevention and Control Measures in Shanghai to Invitees
In order to effectively prevent and control the COVID-19 in Shanghai and protect your health and life safety, please abide by the relevant Chinese laws and regulations and strictly follow COVID-19 prevention and control measures during your stay in China. Now please be informed of the following matters:
1. Please inform the inviting entity of your travel history and health status within 14 days before your entry in China, and the planned mode of transportation, flight number, train or ship schedule, etc. for cooperation with the inviting entity’s prevention and control plan. After entering China, please follow the quarantine regulations and receive nucleic acid test, as required at national and municipal levels.
2. Please take your body temperature every day, wear a mask consciously in public space, do not participate into gathering activities, keep a safe social distance, properly keep your working and living environment clean, and maintain good indoor ventilation.
3. If you feel unwell and see a doctor during your stay in China, please truthfully inform the doctor of your information such as travel history and close contacts within 14 days for cooperation with the epidemiological investigation. Once confirmed, please maintain good mentality and strengthen communication with medical workers for better treatment.
4. Please strictly abide by the Law of the People’s Republic of China on Prevention and Treatment of Infectious Diseases and other relevant laws and the relevant provisions of the Shanghai Municipal People’s Government on COVID-19 prevention and control. In the case of refusal of implementation and behaviors that disturb the order of COVID-19 prevention and control and jeopardize public health and safety, the relevant authority shall have the right to pursue legal responsibility in accordance with the law.
